Banking Details of Beneficiary/Applicant
In the second section of the form, you need to provide your banking details.
- Fill in the Name of the Bank where you want your social grant to be paid.
- Provide the Branch Code of the bank.
- Select the Type of Account from the options: Cheque, Savings, or Transmission.
- Enter your Account Number.
Consent and Confirmation
Once you have filled in all the required information, sign and date the form in the space provided.
