  • In the “Surname” and “Full names” sections, write your last name and first names respectively.
  • In the “Age” section, write your current age.
  • In the “Residing at” section, provide your physical address.
  • In the “Postal Code” section, write the postal code of your residence.

Marital Status

  • Mark the appropriate box with an “X” to indicate your marital status, whether married, unmarried, in community or out of community, civil union, customary union, asiatic religion, never married, divorced, widow/widower, or deserted for more than 3 months.

Spouse/Partner Information

  • If applicable, provide details of your spouse or partner on the back of the form, including their name, surname, and ID document information.
  • State reasons if you do not have any of the required documents for your spouse or partner, such as ID document, decree of divorce, or death certificate.

Disability Grant Affidavit Form: Sources of Income

  • Mark “X” in the applicable box(es) to indicate the type of income/profits received by you, your spouse, or dependant child, if applicable. Options may include salary or wage, profits/withdrawals/benefits from a business or farm (owned or rented), payments from a trust or inheritance, payment from property rights, pension or annuity, ex-gratia payments received, rental income, income from assets (interest/dividends), or income from RSA or international organization.
  • If you or your spouse/partner have no source of income, indicate how you currently survive in the space provided.

Declaration of Assets

  • Mark the appropriate box(es) with an “X” to declare your or your spouse/partner’s assets, if applicable. Options may include immovable property owned/held under leasehold (occupied or not occupied), investments/bonds/loans/outstanding debts due to you, shares/share capital/interest in assets in a company/institution, endowment policies after maturity or cash in hand, or lump sum invested to procure an annuity.
  • If you or your spouse/partner do not own any assets, indicate so in the space provided.

Declaration

  • Declare that all information provided in the affidavit is true and correct to the best of your knowledge.
  • Confirm that you have no objection to taking the prescribed oath and consider it binding on your conscience.

  • Provide the name of the commissioner who will be witnessing your affidavit.
  • Sign the form and provide your thumbprint in the presence of the commissioner.
