Personal Information

Indicate the type of grant you are currently receiving by checking the appropriate box: older persons’ grant, war veterans’ grant, or disability grant. Delete any grant types that do not apply to you.

Confirm that you are not in a state-funded or subsidized institution and that you require regular attendance by another person.

Mark the appropriate marital status box with an “X” based on your current marital status: married, unmarried, in community, out of community, civil union, customary union, Asiatic religion, never married, divorced, widow/widower, or deserted for more than 3 months.
