Affidavit for an Old Age Grant: How to complete the form

The Affidavit for an Old Age Grant is a legal document that requires the applicant to provide sworn statements under oath in order to apply for an Old Age Grant. This form is typically used by older individuals who are applying for financial assistance from the state and need to provide information about their personal details, marital status, sources of income, and assets. Personal Information: Affidavit for an Old Age Grant

Personal Information Under the “Personal Information” section, provide the following details:

  • Surname: Write your last name.
  • Full names: Write your first and middle names.
  • Age: Write your current age.
  • Residing at: Write your physical address where you currently live.
  • Postal Code: Write the postal code of your residence.

Marital Status In the “Marital Status” section, mark the appropriate box with an “X” to indicate your marital status. Choose from the following options:

  • Married: Mark this box if you are currently married.
  • Unmarried: Mark this box if you are not married.
  • In community/out of community: Mark this box if you are married and indicate whether you are married in community of property or out of community of property.
  • Civil Union/Customary Union: Mark this box if you are in a civil union or customary union.

  • Asiatic Religion: Mark this box if you are in a marriage governed by Asiatic religion.
  • Never Married: Mark this box if you have never been married.
  • Divorced: Mark this box if you are divorced.
  • Widow/Widower: Mark this box if you are a widow or widower.
  • Deserted > 3 months: Mark this box if you have been deserted by your spouse for more than 3 months.

If you have more than one spouse, provide details of each spouse on the back of the form.

Reasons for Missing Documents If you do not have any of the following documents for your (ex) spouse or partner, state the reasons in the “Reason” section:

  • ID Document
  • Decree of Divorce
  • Death Certificate

Sources of Income: Affidavit for an Old Age Grant

Sources of Income In the “Sources of Income” section, mark the “X” in the applicable box(es) to indicate your sources of income. Choose from the following options:

  • Salary or wage: Mark this box if you receive a salary or wage.
  • Profits, Withdrawals or other Benefits from a Business/Farm (owned): Mark this box if you receive profits, withdrawals, or other benefits from a business or farm that you own.
  • Payments from a Trust or Inheritance: Mark this box if you receive payments from a trust or inheritance.
  • Payment from Property Rights: Mark this box if you receive payments from property rights.
  • Pension or Annuity: Mark this box if you receive a pension or annuity.
  • Ex-Gratia Payments Received: Mark this box if you receive ex-gratia payments.
  • Rental Income: Mark this box if you receive rental income.
  • Profits, Withdrawals, or other Benefits from a Business/Farm (rented): Mark this box if you receive profits, withdrawals, or other benefits from a business or farm that you rent.
  • Income from Assets (interest/dividends): Mark this box if you receive income from assets such as interest or dividends.
  • Income from any RSA or International Organisation: Mark this box if you receive income from any RSA or international organisation.

Declaration of Assets: Affidavit for an Old Age Grant

Mark the appropriate box(es) with an “X” to declare your or your spouse/partner’s assets.

Options include immovable property owned/held under leasehold (not occupied), immovable property owned/held under leasehold (occupied), investments/bonds/loans/outstanding debts due to you, shares/share capital/interest in assets in a company/institution, endowment policies after maturity or cash in hand, property rights, or lump sum invested to procure an annuity. If you do not own any assets, indicate so in the last box.

Affidavit for an Old Age Grant: Declaration

Read the declaration carefully and sign or provide a thumbprint in the space provided. The deponent’s signature/thumbprint must be placed in the presence of a Commissioner of Oaths.

The Commissioner of Oaths must fill in their name, rank/force number, date, and place, and provide their signature/stamp.

Applicants must meet the following requirements:

  • The applicant must be a South African citizen, permanent resident or refugee.
  • The applicant and child must be resident in South Africa.
  • Must not be less than 60 years.
  • The applicant must not be in receipt of another social grant for him or herself.
  • The applicant and spouse must comply with the means test.
  • The applicant must not be maintained or cared for in a State Institution.
  • The applicant must submit a 13 digit bar coded identity document.
