How to submit an CC3 Application form in Namibia
The CC3 Application form is Application for the restoration of the Registration of a Close Corporation in Namibia. Here are some important things to keep in mind when submitting an CC3 form:

Required Documents To Apply:
- CC1 Application form (plain white or in pastel blue) (triplicate)
- Valid CC8 Name Approval (You get it when your business name is approved)
- Original signed consent letter from a Registered Accounting Officer
- Latest 6 months certified identity documents for Member(s) and Witness
- Additional letter from regulatory bodies such as NAMFISA, NTB, NEAB, NCAQS, ECN, HPCNA (if applicable)
How to complete the CC3 application form
- Shortened form of name and literal translation of name to be left blank.
- Business objectives must be the same as approved on CC8 name application.
- Full identity number is required for Namibian citizens, and date of birth is required for foreigners.
- Registered office email address must be provided on page 1.
- Postal address and registered office address must be completed.
- Accounting officer name and postal address must be completed.
- Member(s) and witness contact details and email address must be provided.
- The body of the Accounting officer letter must correspond with the information in the consent letter.
- Member(s) percentage of interest MUST add up to 100%.
- Financial Year End must be completed.
- Registration number must be left blank.
- Member(s) and witness must sign with a black pen.
- Business name MUST appear on all the pages.
- No alterations allowed on forms (scratching).
- No abbreviations allowed on forms (including initials of persons and town names/suburbs. Insert the town name where it is applicable at all times).
- Do not double-side print the Founding Statement.
- Details of all members on the form must appear on page 10 (members AND witness).
- Copies of certified ID/passport copies are not allowed. The originally certified copy must be submitted.
- Scanned copies of consent letter of accounting officer are not allowed. The original consent letter must be submitted.
Application Fee:
- The application fee is N$150.00.
