Submission Details
- Date: Fill in the current date of submission.
- Contact Details: Provide your personal contact information, including:
- Name and Surname
- ID Number
- Date of Birth

- Telephone Number
- Email Address
- Residential Address
- PO Box
Educational Information
- Educational Institution: Fill in the name of the educational institution where you were funded by NSFAF.
- Years of Study: Indicate the years of study for the program you were funded for.
- Field of Study: Provide the field of study or program you pursued.
- Student Number: Fill in your student number.
- Year of Graduation: Indicate the year you graduated from the program.
Educational Information (if funded for more than one program)
- Educational Institution: If you were funded by NSFAF for more than one program, fill in the name of the additional educational institution.
- Years of Study: Indicate the years of study for the additional program you were funded for.
- Field of Study: Provide the field of study or program for the additional program.
- Student Number: Fill in your student number for the additional program.
- Year of Graduation: Indicate the year you graduated from the additional program.
Employment Information
- Date of First Employment: Provide the date of your first employment after graduation.
- Current Employer: Fill in the name of your current employer.
- Work Telephone Number: Provide a telephone number where you can be reached at work.
Submitting Your Form
- Submit to any NSFAF official, at the NSFAF Head office or at the NSFAF regional offices.
- Email to recovery@nsfaf.na.
- Post to: P O Box 23053, Windhoek.
NSFAF Recovery Division will contact you regarding the repayment process.
