How to Complete the CC2 Form for Amended Founding Statement

Steps to Complete the CC2 Form

Gather the necessary information

Before filling out the form, you should have the following information at hand:

  • Registration number of the corporation
  • Dates of commencement of change
  • Full name of the corporation
  • Previous name of the corporation (if applicable)
  • Literal translation of the name (if applicable)
  • Shortened form of the name (if applicable)
  • Description of the principal business
  • Date of the end of the financial year
  • Postal address

  • Address of registered office (not a post office box)

Fill out Part A

Part A of the CC2 form requires you to provide basic information about the corporation, including the dates of the change, the name of the corporation, and the principal business. You will also need to provide the postal address, email address, and address of the registered office. Finally, you will need to include the name and address of the accounting officer, along with their written consent to the appointment.

Fill out Part B

Part B of the CC2 form requires you to provide additional information about the accounting officer, including their full name, membership/practice number, and the name and address of the association or body of which they are a member. You will also need to attach a written consent to their appointment and include the signatures of the previous and current accounting officers.

Notes on Completing the Form

  • Form CC2 should be written in block capitals or be typewritten, lithographed, or printed in legible characters with deep permanent black ink, and lodged in triplicate.
  • Any changes made will be effective from the date of registration or upon a later date mentioned.
  • If a person signs on behalf of a member, a power of attorney must be attached.
  • Minor children or persons under legal disability must be assisted by their parents, guardians, or representatives, and their capacity must be stated.
  • Particulars that do not change should also be furnished.
  • No fee is payable in respect of any changes in particulars under Parts B and C.
  • New members must personally sign the form.
  • Form CC2 that does not comply with the requirements of the Act, regulations, or these notes will be rejected.

Once you have completed the CC2 form and reviewed the notes on page 2, you can submit it to the appropriate authority. Remember that the form must comply with the requirements of the Act, regulations, and notes, or it will be rejected.
