Requirements for Applying for a Namibian Passport:
- Duly filled-in application form
- One colour passport-size photo, signed by the Applicant on the back (Not required at Windhoek Office)
- Your current passport with a good quality certified photocopy of the personal page of your current passport (if available)
- Old Passport, or Police Affidavit if lost/stolen (for previous applicants)
- Namibian Birth Certificate (if applicable)
- Namibian Identity Document (ID)
- Namibian Citizenship Certificate (if applicable)
- Certified copy of Marriage Certificate (for married women who have not updated their ID to husband’s surname and want his surname on their passport)
- Certified copy of Divorce Certificate (for divorced women wishing to revert back to their maiden surname)
- Original parent’s identification (for minors under 16). Any child over 13 must be present for signature.

- Birth Certificate or ID showing correct information
- Relevant good quality certified photocopies of the above documents should be submitted with your application.
Reporting a Lost or Stolen Passport:
If your passport is lost or stolen, it must be reported to your local Police. You should obtain a document from the Police to say that the passport has been reported missing, and submit this document when you apply for a new passport.
