Telecom Namibia provides telecommunication services in Namibia. If you are a Telecom Namibia customer and wish to apply for TN Mobile Caller ID or Caller ID Restriction, you will need to fill out an application form and submit it with the required documents.
How to Fill out the Application for TN Mobile Caller ID or Caller ID Restriction
TN Mobile Caller ID Application Requirements
To apply for TN Mobile Caller ID or Caller ID Restriction, you will need to attach the following documents to your application form:
- Copy of ID or Passport document
- Copy of Pension Card (if applicable)
- Copy of Marriage certificate (if applicable)
- Copy of Business/ company Registration Certificate (if applicable)
Personal Details
The application form requires you to provide your personal details, including:
- Title (e.g. Mr, Mrs, Dr)
- Full name(s)
- Date of Birth
- Occupation
- Identity/Passport number
- Citizenship
- Postal Address
- Contact details (telephone number, cell phone number, fax number, and email address)
- Marital Status
- Spouse name and Spouse Identity/Passport number (if applicable)
- Next of kin/Friend name and contact number

On the application form, you will need to tick the applicable service that you require:

The normal terms and conditions still apply when you apply for TN Mobile Caller ID or Caller ID Restriction.
Payment Undertaking (Personal)
If you are applying for TN Mobile Caller ID or Caller ID Restriction as an individual, you will need to sign the payment undertaking section of the application form. By signing, you are declaring that the information provided in the application form and copies of attachments are true and correct, and you accept and agree to the terms and conditions of the contract.
Payment Undertaking (Business)
If you are applying for TN Mobile Caller ID or Caller ID Restriction on behalf of a business, you will need to sign the payment undertaking section of the application form in your capacity as a representative of the business and in your personal capacity. By signing, you are declaring that the information provided in the application form and copies of attachments are true and correct, and you accept and agree to the terms and conditions of the contract.
You will also be required to acknowledge and agree that by your signature, you bind yourself as a surety and co-principal debtor in solidum with the applicant in favour of Telecom Namibia Limited for the due payment by the applicant of all amounts which may now or at any time hereafter become payable by the applicant to Telecom Namibia Limited.
